What Does Memory Care Look Like?

We know cognitive challenges can affect everyone differently, and our residents are provided with customized care plans based on Teepa Snow’s GEMS memory care model. We can identify the unique level of care your loved one needs and customize every part of their experience with us.
Teepa Snow’s GEMS® Model
Our community uses proven memory care techniques to care for our residents, and we help our residents’ families learn more about effective communication strategies for engaging with their loved ones.
Our team takes inspiration from Teepa Snow’s Positive Approach to Care and the GEMS® model. We work to understand each of our residents’ strengths and abilities, offering adaptable care plans based on 6 levels of care, or “gems.”
Sapphire
A sapphire brain functions normally, is flexible and organized, and pairs well with other GEMS. Individuals in a sapphire state are known to be incredible care partners for those with dementia on account of their adaptability.
Emerald
In the emerald state, people may lose clarity of thought and struggle with communication. Despite these changes, they may attribute some of their challenges to others.
Family members and caregivers often find more success in providing support when they’re willing to adapt to the situation and work with their loved ones.
Diamond
Those in a diamond state often stay sharp and clear but may seem more rigid. They often have many cognitive abilities but show a certain inflexibility to daily challenges or changes. They can still shine despite their aversion to change.
This state may make it challenging for loved ones to spot the presence of dementia, as it can lead to a blend of past and present behaviors.
Pearl
Those in the pearl state often have reflexes that are constantly active, failing body systems, and deteriorating gross motor skills. Despite these setbacks, the right support system can still help individuals experiencing these effects thrive, revealing the remarkable pearl within them.
Ruby
In the ruby state, a person may notice a decline in their fine motor skills, which may impact vision, speech, and the ability to chew and swallow easily. However, their strength is often unaffected and they can often still perform larger movements.
